If you are unable to open and / or navigate to the next page in the self form but do not receive any error messages, please try to open the link you received by e-mail in another browser (Chrome, Safari), and / or on another device (computer, telephone).

If the problem persists, please send information about which browser and device has been used to your contact person for the form, or to form support.

Note that some email clients may shorten the link and remove necessary authentication information.
